have been living with a serious and enduring mental health diagnosis for at least two years
caring for someone with a serious and enduring mental health diagnosis.
Why are we asking you to complete this survey?
We want to gain an understanding of your experiences using NHS mental health services, good, bad or in the middle and how services have impacted everyday life and wellbeing.
Please note In this survey, we use the term serious and enduring mental health condition to mean a mental health condition that has been diagnosed at least 2 years ago and has had a long‑term impact on someone’s life, often requiring ongoing NHS mental health support.
